The Health Equity, Policy & Planning team celebrated many successes in community engagement, equity infrastructure development, civic engagement and other domains throughout 2024. Below are some of the team’s achievements: 

• The Community Health Improvement Plan from planning to implementation with three priority area work groups – Social Determinants of Health, Mental Health and Access to Health Care
• Implementation of the first Public Health, Policy & Planning Equity Plan
• Community-focused Public Health 101 Workshops were launched
• Community partners were engaged to build the Youth Voter Project curriculum
• The CalFresh Healthy Living team continued partnering with clinical providers to address food security through Food as Medicine and increase park use through the Mariposa Program
• The Health Equity, Policy & Planning team remained committed to professional development
• Team members have continued to explore growth via conference attendance
