Community health workers (CHWs) are an important part of the health workforce. They support community members through health coaching, advocacy, and resource navigation. Research has shown that well-designed CHW programs can: 

• improve chronic disease control and mental health 
• promote healthy behaviors 
• reduce hospitalizations 
• increase participation in primary care
