Toolkit for Medicaid Reimbursement of Community Health Worker Services
Community health workers (CHWs) are an important part of the health workforce. They support community members through health coaching, advocacy, and resource navigation. Research has shown that well-designed CHW programs can:
- improve chronic disease control and mental health
- promote healthy behaviors
- reduce hospitalizations
- increase participation in primary care
